What CMS measures about every hospital
The Centers for Medicare & Medicaid Services tracks these categories for every Medicare-certified hospital. We explain what each one means and show you the numbers — we do not score hospitals ourselves.
Death Rates
How often patients died within 30 days of being treated for certain conditions. Lower is better.
Safety & Infections
Infections and surgical complications that happened during a hospital stay. Lower is better.
Unplanned Returns
How often patients had to come back to a hospital within 30 days after being discharged. Lower is better.
What Patients Said
Results from surveys of patients after they went home. Higher is better.
Timeliness of Care
How quickly patients were treated and whether the hospital followed recommended steps of care.
The 1-to-5 Star Rating
CMS combines the categories above into a single star rating. Not every hospital gets one — and that is normal, not a warning sign.
How to use HospiGuide
Hospital data rewards a little context. Three things worth knowing before you compare.
Distance matters more than stars
For a heart attack or stroke, getting to any hospital fast beats travelling further to a higher-rated one. Use ratings to choose between realistic options, not to rule out the closest emergency room.
Look at what you actually need
An overall rating averages everything. If you are having a knee replaced, the hip/knee complication rate tells you far more than the summary star.
"Not reported" is not a red flag
Small and rural hospitals often treat too few patients for a measure to be statistically meaningful, so CMS withholds it. We show you the reason instead of an empty cell.
Where our data comes from
Every number on this site is published by CMS through Care Compare and reproduced without modification. We add plain-English labels, explain which direction is better, and compare each hospital with the national median — but we never calculate our own scores, accept payment for placement, or publish user reviews. Current data was released by CMS on August 13, 2026. See the full list of datasets →
